Employee Handbook Acknowledgement Form

I have received my copy of the SDABA Employee Handbook (the “Handbook”). I understand that I should consult my supervisor, Human Resources, or Officer of the Company regarding any questions not answered in the Handbook.

Because the information, policies, and benefits described here are necessarily subject to change, I acknowledge that changes to the Handbook may occur. All such changes will be communicated to the employees, and I understand that revised information may change or eliminate existing policies. Only an Officer of the company can adopt any revisions to the policies in the Handbook.

I have entered my employment relationship with this company voluntarily and acknowledge that there is no specific length of employment. Accordingly, either I or the company can terminate the relationship at will, with or without cause or notice, at any time.

Furthermore, I acknowledge that the Handbook is neither a contract of employment nor a legal document. I have received the Handbook, and I understand that it is my responsibility to read the Handbook and any revisions made to it.
